NURSING

  • Help promote a patient’s independence by establishing patient care goals, and by teaching patients, their families, their relatives and their friends about the patient’s condition. They also offer medications and self care skills, as well as answer questions.
  • Use clinical judgement to protect, promote, and optimize the health of the patient. They also help to prevent illness and injury, alleviate suffering, and advocate in-home health care for individuals, families, communities and populations.

OCCUPATIONAL THERAPY

  • Help people of all ages to improve their ability to perform tasks in their daily living and working environment.
  • Assist the elderly to lead more productive, active, and independent lives through a variety of methods, including the use of adaptive equipments.
  • Help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function.
  • Assist clients in performing activities of all types ranging from using a computer to caring for daily needs such as cooking, eating, dressing, and bathing with or without assistive devices to perform each task safely.

PHYSICAL THERAPY

  • Evaluate components of movement including muscle strength, range of motion, flexibility, balance, posture, body mechanics, coordination, endurance, and general mobility (walking, stair climbing, getting in and out of bed/chairs).
  • Seek to identify and maximize quality of life and movement potential through prevention, intervention (treatment), promotion, and rehabilitation.
  • Are experts in the examination and treatment of people with cardiovascular, musculoskeletal, and neuromuscular diseases, focusing on conditions and problems that undermine patients’ abilities to move and function effectively. It provides services that help restore function, improve mobility, relieve pain, prevent permanent disabilities and limit permanent disabilities.

HOME HEALTH AIDE

  • Help elderly, disabled, and ill persons live in their own homes instead of in a health facility. Others help discharged hospital patients who have relatively short-term needs.
  • Are usually registered nurses, therapists, or a social workers in home health care agencies who assign specific duties and supervise home health aides. Aides keeps records of services performed and of clients’ condition and progress. They report changes in the client’s condition to the supervisor or case manager. Home health care aides also participate in case reviews, consulting with the team caring for the client-registered nurse, therapist and other health professionals.

REGISTERED DIETITIAN

Whether a client is admitted for short term or long term care, the dietitian plays a vital role in a patient’s care. They not only evaluate the overall menu, they also monitor the nutritional needs for at-risk residents. These patients could be at-risk for skin breakdown leading to decubitus ulcers, weight loss, dehydration or diabetes. The Dietitian also monitors other patients to watch for developing risk factors.

MEDICAL SOCIAL WORKER

Make important contributions to society by working to improve the lives of individuals, families and groups. The exact role a social worker plays depends on the work setting, as social workers provide a broad range of services to help people in need. They offer direct care services, helping people cope with everyday problems, and clinical services.

What Is Home Health Care?

Home health care involves a range of health care services that can be provided wherever home may be. Also known as ‘skilled care,’ home health care is designed to help you gain independence and remain safe at home as an active community member.

People turn to home health care for help with:

  •   Transitioning from a hospital or nursing facility, to remain stable and stay at home
  •   Managing chronic conditions like diabetes, CHF, or COPD
  •   Rehab after joint replacement or other surgery
  •   Adapting to new mobility challenges, and home safety and falling hazards
  •   Caregiver support and strategies for living with cognitive decline

Who Pays for the Services?

Our staff assists families and patients to determine eligibility and available payment options. Most DynaFlex Home Health Care services are covered by Medicare,  Medicare Advantage BCBSM PPO Plus Blue, Auto Insurance, and many Private Health Insurance programs. Our clients may choose to pay privately as well.

Are you or a Family Member a Candidate for Home Care?

Being hospitalized is not a requirement for Home Care services. DynaFlex Home Health Care accepts referrals from a member of the family, the physician, nursing home, the client, or a social worker.

When is Home Care Right?

Home care is right when:

  • A person is about to be discharged from a nursing home or a hospital and needs skilled home care.
  • A person recovering from outpatient surgery and needs short-term skilled home care.
  • While under the care of a physician, a person who is medically unstable and requires assisted living in their home and needs skilled home care.
  • A person is newly diagnosed with illness and needs skilled home care.
  • A person who requires therapy services and needs skilled home care.
  • A person needs education on a change of medications and needs skilled home care.
